Our long-standing, West Lothian based client is current seeking a locum Social Worker for a reablement and care packages role with their Assessment and Prevention team. Working with vulnerable adults in West Lothian’s community you will undertake reviews of service users who have been receiving a variety of supportive services. You will work closely with these individuals to gain a comprehensive understanding of their current needs with respects to their eligibility for the type of support that is both available and is being received.

As a Social Worker in West Lothian working within an Assessment and Prevention team, you will use the information gained from the assessments of clients to create revised care and support plans. In addition to this you will signpost clients in West Lothian to appropriate support services, for instance through charities and will keep the promotion of independence at the forefront of your mind.

You will consider where technology could be used to support vulnerable adults, focussing on the individual’s current strengths and how to sustain and encourage these further.

In order to be considered for the role of Social Worker in West Lothian, you must:

· Have a professional qualification in Social Work
· Have experience working in reablement
· Membership with the PVG Scheme and the willingness to undertake an update
· Have knowledge of Health & Safety, lone working & how it relates to working within clients’ homes
· A UK driver’s licence and access to a car
· Have genuine empathy and concern for vulnerable individuals and the passion to try and bring about positive change.
