An exciting opportunity has arisen with one of our Public Sector partners for a Community Psychiatric Nurse within their Community Care team in the North of Scotland. This opportunity will give the ideal candidate the chance to help support those patients with specific needs within a community nursing environment at a time when there is a great need for consistency within the team.
This community psychiatric team services a large geographical area and touches the lives of many vulnerable individuals in their own homes and supported living environments.
As a Community Psychiatric Nurse, you will:
· Work as part of a multi-disciplinary team (MDT) to provide a clinical nursing service to individuals who have been identified as having mental health issues.
· Form and maintain effective working relationships with relevant services and agencies providing care for people with mental health issues.
· Provide nursing intervention for people experiencing mental illness and mental disorder.
· Provide support and education about mental health for carers and relevant others.
· Assess the needs of people with mental illness and agreeing recovery-based treatment plans with individual patients, providing nursing intervention in home and community setting according to individual’s needs.
· Work within the Multi-Disciplinary Team in partnership with GP’s, Hospital and Community staff, Community Support Services, fellow health and social care professionals alongside patients with complex and overlapping health and social needs.
· Contribute to operational and strategic planning decisions about mental health.
To be considered for the role of Community Psychiatric Nurse, you will need to have:
· Current NMC Registration
· 6 months Post Registration Experience
· Good effective communication skills, ability to listen, respond and react in appropriate manner.
· Approachable, friendly, tolerant, tactful, supportive and diplomatic
· Willingness to be proactive in changing practices and service redesign
