Description

An urgent opportunity has arisen with one of our Public Sector partners for a District Nurse within their Community Care team in the West of Scotland. This exciting opportunity will give the ideal candidate the chance to help shape the nature of community nursing in the area at a time when there is a great need for consistency within the team.

This community care team services a small geographical area and touches the lives of many vulnerable individuals in their own homes. As a community care service, there is a strong local drive to ensure services continue to demonstrate a high level of support for vulnerable service users.

As a District Nurse (RGN) with this community setting, you will:
· Provide a community nursing service to the housebound population in conjunction and collaboration with HSCP colleagues
· Work collaboratively with all team members to provide effective and holistic nursing care to a designated caseload
· Be the clinical expert who will be accountable for case management and nursing activity within the caseload, and lead, manage and support the integrated team.
· Work collaboratively in both a planned and responsive manner to ensure nursing skill and expertise is afforded to residents of the HSCP
· Make daily assessments of patients – physiological, psychological, social needs
· Managed care planning for your allocated patient group – appropriate nursing care, risk assessment, moving and handling
· Implement care plan – nursing actions, patient involvement
· Evaluate care plan – how well interventions have worked.
· Assess and advise on discharge criteria- suitability to return home, discharge liaison

To be considered for the role of District Nurse (RGN) with this community setting, you will need to have:
· Current NMC Registration
· 6 months Post Registration Experience – preferable within the community
· An interest in Palliative Care and Management of Long Term Conditions / Anticipatory Care alongside associated technical skill & ability is considered advantageous. E.g. enteral feeding T34 Syringe Pumps Evidence of their ability to work in Partnership with Multi-disciplinary team and Inter-Agency personnel
· Good effective communication skills, ability to listen, respond and react in appropriate manner.
· Approachable, friendly, tolerant, tactful, supportive and diplomatic
· Willingness to be proactive in changing practices and service redesign
